The Envelope, Your Personal
Survivor Info Document
Nobody likes to think about it
much, but when we die, our survivors will need a lot of information that
relates to our personal finances: pension, health insurance and Welfare
Fund benefits, banking and investment accounts, and other data. Often
this information is scattered among desk drawers, sock drawers and
various PC computer files, making a difficult task that much harder for
our family.
To help you organize essential
survivor data, the Welfare Fund has created a special document that
includes important phone numbers and website addresses for your pension
system, NYC health benefits program, the Welfare Fund and more. It will
help you to record the benefit programs you participate in, the accounts
you carry, and the agencies that should be notified upon your death.
